	Robert Shearman <rob at codeweavers.com>
	- Make struct oletls ref counted so that it is only detached from the
	apartment on the final CoUninitialize.
	- Decrease the size of the crit sec on destroying an apartment - it is
	only needed for touching the apartment list.
	- Small cleanups.
