While you are just at it, shouldn't the first char in SystemBuffer be skipped?

"packet.reportBuffer = irp->AssociatedIrp.SystemBuffer;"

Also, it looks like the first assignment of

"irp->IoStatus.Information = 0;"

is not really required, its also assigned later.
