[PATCH vkd3d 5/5] vkd3d-shader/hlsl: Correctly calculate offsets for array elements.

Hi,

I think the patch is mostly ok, but both hlsl-struct-array.shader_test 
and hlsl-nested-arrays.shader_test are failing here, with basically the 
same error:

Could it be that you tested them on a different branch?
