More changes for Shlwapi

With these changes the Outlook Express 4 gets further and will even display
the .gifs in the OE entry level (top level folder).

  dlls/shlwapi/shlwapi.spec
   - Added stdcalls for _18,_21,_22,_26,_27,_28,_34,
      UrlCombine{A|W}

  dlls/shlwapi/ordinal.c
   - Added stubs for _18,_21,_22 which seem to be used only
     by native shdocvw.dll
   - Implemented _26, _27, _28, _32, and _34 (more isw... functions).

  dlls/shlwapi/url.c
   - Stub UrlCombineA and initial implementation of UrlCombineW.
       (Note: not complete yet)
