Some more Shlwapi ordinal routines

With these changes (and the separate ones to reg.c and path.c), OE5 will
start up with builtin shlwapi and comctl32. You can even open a new mail
message. Still however cannot access the databases.

Oh well.

    - New handle pointer to access to support Version.dll used
      ordinals 350, 351, and 352.

  dlls/shlwapi/ordinal.c, ordinal.spec
    - Forward ordinals 40, 41, 143, 362 to appropriate routines.
    - Implement ordinals 155, 350, 351, 352, 418, 436.
    - Implement part of SHGetInverseCMAP.

  dlls/shlwapi/ordinal-cmap.h   <--- new file
    - Has the 32k bytes that are returned by SHGetInverseCMAP.
