bug#344 - commdlg filedlg issues

The attached patch was put on wine-dev with no comments so submitting.
Resolves all the issues in bug#344 as far as I am aware:

Fixed file dialog:
 InitialDir handling and InitialFile handling now agrees with MSDN
description, and testing on win2000 appears to agree
 Filename field not cleared when directory selected/clicked on
 OFN_NOVALIDATE field no longer causes traps, and appears to behave as per
w2k
