PATCH: provide MSVCRTD.DLL debugging C runtime DLL (second try)

the attached patch adds an implementation of the MSVCRTD.DLL debugging
C runtime DLL. The version of the patch provides the DLL's functionality
by fowarding calls to MSVCRT.DLL (as suggested by Alexandre Julliard).

you will need latest CVS to use this, otherwise "register" functions can't
be forwarded.
