SMB changes (take 2)

Once more, with feeling:

ChangeLog:
- implement both raw-mode (port 445) SMB and
SMB-over-NetBIOS
- separate file functions from SMB functions
- remove fd_ops and (unused) dialect from wineserver
smb
- general SMB fixes
