Sync (29): msvcrt cpp

Hi,

The remainder of the changes to msvcrt cpp objects. Note that
directly accessing the type info of our exported classes through the
vtable is not possible without the hack i used in the first
submission of this patch; it remains to be seen if that is a problem
in practice.

    Use exception as the base for all exception derived classes.
    Implement type_info methods correctly.
    Add static RTTI for exported objects.
    Throw exceptions when run time casts fail.
