this patch fixes two bugs:
- the first one has been reported by Rémi Assailly, and was about some strange 
errors in playsound. They happened in loop mode, when the sound was small enough 
to fit in a single waveheader.
- the second is an array indexing out of bounds found by Intel compiler. Handy.
