Includes: ntdll

Hi,

Start of the slimmer #include removal patches, posted per-dll to
avoid patch dependency problems.

Drop any that don't apply cleanly and I'll recreate.

ntdll: 21 includes removed.

    Remove uneeded headers to reduce uneeded rebuilds.
