Toolbar: Better Hot Item Handling

Hi,

This patch generalises all of the hot item changing into the
SetHotItemEx function. It fixes a bunch of bugs with buttons staying hot
after clicked, drop-down buttons not being redrawn properly and not
always sending notifications to the parent. Also included in this patch
is a small documentation update and a fix for DrawMasked to use the
right image list and bitmap index.
