Dbghelp: enhance PDB support

This (huge) patch does:
- apart from restructuring the code (COFF handling goes to new coff.c 
file, Codeview and PDB defines and types to new mscvpdb.h file)
- rationalize the naming and coding convention of Codeview and PDB 
defines & types
- implement support for import in pdb file (needed when most of the 
types are stored in a vcX0.pdb file)
- implement support for the latest PDB format (aka DS or RSDS)
I suspect, this is still not perfect, but there's still a lot of debug 
code in it, so we should catch the unsupported forms rather easily
