[wined3d] intergrate vertexshaders into drawing pipeline [vs part 5]

Hi,
    this patch intergrates hardware vertex shaders into the drawing pipeline.

The following demos now work:
HLSLWithoutEffects
dx9_cg_lighting
dx9_cg_simple
dx9_cg_anisotropic_lighting

There's a known regression that some colours may be wrong (red and blue are
swapped), this will be corrected in a different patch.

This is part 5 of the vertex shader patches.
