[d3d9] final tidyup.

Hi,
   This patch tidies up d3d9 and finalizes passing d3d9 calls to wined3d for now.

Changes mainly add a some comments and remove unused declarations, includes etc... from
d3d9_private, s small bug was fixed in surface.c and a missing release call has been added to
swapchain.


The debug channel is now d3d9 for all calls to d3d9.
