[AppDB] Prepare for smaintainer request on app submission

Prepare the maintainer system for super maintainer requests on app sumission:

- If a user has sufficient privileges, he is automatically approved as a 
maintainer.  For now, only admins have this right.
- Do not display maintainer entries that belong to a queued application; they 
are handled automatically
- Make sure using unQueue() on an approved entry does not delete it
