[AppDB] Allow maintainer requests on version submission

Add a checkbox so the user can send a maintainer request along with a new 
version.  The request is automatically processed and hidden from the 
maintainer queue, just as with super maintainer requests on app submission.

If, when submitting an application, both the maintainer and super maintainer 
boxes are checked, and error is printed explaining why this is wrong.  It 
would be possible to just ignore the maintainer request and process the super 
maintainer request, but the ticking of both boxes signals a lack of 
understanding of the maintainer system, and is best clarified this way.
