[AppDB] - maintainer cleanup

Start to move maintainer related functions into maintainer class and separate 
maintainer related queries/logic display code.  Also move maintainer related 
code from the user class where much of the existing maintainer related code 
was put.

Move maintainer related unit test code into test_maintainer.php and fix up the 
unit test to work with the new standardized maintainer class.
