commdlg: file name and extension offsets are not guaranteed to be the same in W->A conversions

commdlg: file name and extension offsets are not guaranteed to be the 
same in W->A conversions especially with Composed and uncomposed unicode 
characters. So calculate the A versions based on the A strings.
