inkobj: Implement IClassFactory

This patch has been redone so that it will apply on top of my
previously submitted patches.  I have based this almost exactly on
mshtmls iclassfactory implementation, the main difference being header
declarations to allow it to be placed in a file separate from
DllGetClassObject.

I have fixed the header problems that plagued earlier versions of this patch.
