[3/5] wined3d: Fix the nvrc implementation of WINED3DTOP_MULTIPLYADD and WINED3DTOP_LERP

These were originally coded against MSDN which, if not just plain
wrong, turns out to be misleading. Tests for correctness of this
change and the other texops follow.
