This is a resend of my patch series from a few days ago
patches 1-5 merge index and vertex buffers; That used to be a big patch, and
I've kept the wined3d_buffer_desc structure in place.
(There is still some work to do with that structure, but that goes beyond this
Patch 6-10 are the rebased other patches
