ddraw: Separate reference counting for IDirectDraw7 and IDDS4 from IDDS3, IDDS2 and IDDS

Resending old patch by Michael Karcher (2008-08-10).
4 tests from ddraw dsurface start to pass even though it's still not
correct (just that this splitting is almost done already an needs just
finnishing touches).
Further patches will separate IDDS refcounting completely.

As this is my first try with patch please write what is wrong with it.
I won't find obvious mistakes :)
