dsound: Correct deallocations when DuplicateSoundBuffer fails.

Hi,

this is the third and last in my current series of dsound patches.
Using the new DSOUND_CopyFormat, it depends on the previous patch.

It fixes the following:
 - device->buffer is a shared resource with refcount;
 - ppdsb not zeroed in last case;
 - also, I found it illogical to call RecalcFormat before CopyFormat.
