msxml3: Create additional node for xml declaration

  Fixes bug 20223 and lets app from bug 4286 to get further.

The only trick we need for that is to skip this new node from being 
written to file/stream on document saving, cause a declaration node will 
be written anyway and there's no way to disable it.
