3/2 wined3d: move palette uploading code over to blit_shader

This patch makes the palette code in surface.c already a bit nicer by
moving the palette uploading to blit_shader. The shader is now
activated from blt_to_drawable and it is also deactivated from there.
Before the shader was never disabled which was not nice. A future
patch will also update the GL_EXT_paletted_texture code.
