[PATCH 7/7] quartz: Create and hold onto a preferred allocator for IAsyncReader::RequestAllocator.

Description:
    The custom quartz filter used by Fallout 3 and Fallout New Vegas
currently fails to play background music (Bug #24478).  This patch partially
fixes the issue that results in these problems, allowing the background
music to play on the title menu (though not while playing the game).  This
patch is inspired by Eduard - Gabriel Munteanu's attempted fix, originally
submitted here:

    It is worth noting that applying this patch exposes a problem with
commit 64b14c0ecdd13530a92ff636c451523a99253a0b interacting with
PulseAudio.  When using PulseAudio (though not with PulseAudio disabled or
the commit is reverted) then no audio will be heard except at the title
menu.
