Hi,

actually, the 3 patches are completely independent.

This is usual bunch of "don't touch header past notificationcallback"
that I thought I had corrected for all cases and drivers years ago.

MidiInDev[wDevID].*= should be ready before thread creation.

The bEnabled test is a dupe.
