Add initial support for console Insert Mode

This patch adds initial support for console Insert Mode.

1. Insert Mode is enabled by default.
2. The user backend dialog now indicates whether Insert Mode is enabled or disabled.
3. Inser tMode is set in the registry.
4. The appropriate console mode is now restored when console-based applications are loaded. (This does not apply to 'wineconsole cmd'. This case must be handled separately.)
