[2/2] server: Remove output_debug_string request. (try 2)

These two patches replace 105196. Patch 105197 is still valid and can be
applied afterwards.

As the client side is now responsible for converting exceptions to debug
string events, we don't need these structures/functions anymore and can
safely remove them.

NOTE: Requires ./tools/make_requests. Please note that the changes in
server/trace.c below are not autogenerated and thus have to be part of
the patch.
