v4: Remove the surface->ddraw->wined3d_swapchain check.
The thinking is the following: The swapchain is independent of the
surface. It is supposed to be always there when the ddraw object is
around. The only exception is when ddraw_create_swapchain fails in
set_cooperative_level, but that's an ERR condition anyway.
In theory surface->ddraw may not be around in case of ddraw1 and ddraw2.
But in that case ddraw_surface_wined3d_object_destroyed would crash
anyway. Presumably destroying ddraw before destroying the surfaces
crashes on Windows too.
We do not create a ddraw surface for the wined3d frontbuffer, so I don't
see how ddraw_surface_set_palette can be called during
ddraw_destroy_swapchain.
Finally, there's the potential issue of a cooperative level switch while
a primary surface with a palette is in use. In this case recreating the
swapchain will set wined3d's palette to NULL. I have some tests for this
case, but I'll defer sending them until Henri is done with his lost
device work. The short summary: Windows doesn't restore the palette
properly either.
v3: Always update the DIB color table in the GDI present code.
I benchmarked the impact of the SetDIBColorTable call, and it is fairly
small. Something like 1.5% in an application that does nothing but flip
a 640x480x8 surface again and again (1040 fps -> 1027 fps).
The change in the ddraw tests is necessary because the undefined palette
black collides with the expected black from the P8->RGB blit. Thus the
test succeeds in todo_wine, but for the wrong reason.
v2: Control the front buffer redraw from ddraw.
Note that at the moment both GL P8 present paths always re-upload the
palette.
