ntdll: Reserve TLS slot 0 for broken apps that compare index to 0 instead of TLS_OUT_OF_INDEXES.

Spotted by Anastasius Focht in bug 20466. Tests are rather weak, but
they are enough to reveal the problem on Wine and I don't see how they
could be improved.
