d3d11: add a stub for D3D11CreateDeviceAndSwapChain (try 2)

This allows Game Dirt 2 demo to load.

Change the UINT to use %u.
Changed return E_NOTIMPL.

The Dirt2 Demo uses this function to determined if DX11 is supported, 
and when it fails it falls back to using DX9.
