include: Add stuff related to WSAPoll() (try 5)

try 5:
Ensure that when port.h is used we don't try to define POLL stuff,
this solves the warnings in mingw and still makes ws2_32 useful.

try 4:
Use Sebastian's idea of not naming the struct since only WSAPOLLFD is
used anyway.
