I haven't looked at the comment, but I think that "app.exe" is meant to
be an example, and "app.exe" is meant to be replaced by the actual name
of the executable in question (i.e.
Software\Wine\AppDefaults\Starcraft.exe\DirectSound), in order to make a
per-application setting that will relate only to that particular *.exe,
and not all the others you may run.
