It took me a while to get to this, but does the attached patch help? It may not
be fully correct; needs tests.

Also, how was hull0.fxc created? It has an invalid DXBC checksum (all zeroes).
Currently vkd3d-shader does not validate DXBC checksums, but it will in the
future.
